HOW
DO I....
OBTAIN
A REPEAT PRESCRIPTION ?
Repeat prescriptions
authorised by your doctor may be obtained by completing the tear-off slip on the
right-hand side of your prescription. Your slip should be submitted to the surgery
by hand, post or fax (01267 234422). Prescriptions can also be requested by telephoning
the dedicated prescription line between 9.00am and 3.00pm on 01267 236242. 24 hours' notice is required to process your requests. Please
supply a stamped addressed envelope if you wish it to be posted to you. All local
